We work with many organisations to offer a more holistic package of care and support to our patients. One such organisation is the Southwark Wellbeing Hub. They offer a wide range of services:

What Is End Of Life: End of life care - NHS (www.nhs.uk)
Are you a Carer?
Helpline Information: https://www.nhs.uk/social-care-and-support/ Advice for carers and the people they are looking after on claiming a whole host of other benefits unrelated to their disability or caring. If you are please let us know (contact) – we may be able to help you.
If you're providing a caring role for a family member or friend you may be able to get support. You can get advice and support from Southwark Carers and Mobilise.
You can also request a carer's assessment. It is not a test of your caring abilities. It's for us to assess what support, if any, could help you in your caring role via Friends, Family and Self-Referral Form
This support could include:
- education and training
- practical help in the home
- support for your mental and physical wellbeing
You do not need to be living with the person you look after, or be providing full-time care.
You can also refer someone else for a carer's assessment.
